When we must share your information

We may have to share your information if:

• A serious crime has happened.
• Someone is in danger.
• A court tells us to.
• We must report certain diseases or injuries.

Emergency calls

• If you call 999, the call is recorded.
• This helps with safety, training, and checking quality.

Surveys and research

• We may send you a survey about your care.
• You can choose to take part in interviews or research.
• You can change your mind at any time.

If something goes wrong

We will tell you if your information is lost or used wrongly.

• This is called Duty of Candour – being honest when things go wrong.
