We (EEAST and EEAST Charity) often use photographs, images and quotes to promote the work of our organisation. To use what is created in the project you are involved in, it is necessary for us to gain written consent so photographs and videos of yourself can feature in publications.
When using images of people, where any individuals feature prominently and are clearly identifiable, this consent form must be signed by those who can be clearly identified. Images of children require the consent of a parent or guardian.
For us to use any of your imagery, you must first give consent that you are happy for us to use and store your image. This consent should be given freely and without any pressure. The Trust will store your imagery securely and only the appropriate members of staff will have access to your image. Your information will be stored for this purpose for two years and then safely destroyed.
Please remember that you have the right to be forgotten or to withdraw your consent at any time.
If you wish to do so, please contact internalcommunications@eastamb.nhs.uk with your request. Please visit our website for more information on how we store and use your data.
