1. How many serious untoward incidents were recorded by your organisation in (i) 2018/19 and (ii) from 1/4/2018 to the 17 June 2019, where the degree of harm was recorded as “severe” or “death”?

How many serious untoward incidents were recorded by your organisation in (I) 2018/19 and (ii) from 1/4/2018 to the date of this e-mail (17 June 2019), where the degree of harm was recorded as “severe” or “death” AND where there was either a delay in the ambulance arriving at the incident scene or a delay in the ambulance handing the patient over to a hospital?

3. Please provide me with a brief summary of each of the incidents in Q.2 stating (i) the degree of harm recorded and (ii) a brief summary of the incident including how long the delay was and why the delay occurred and the consequences for the patient of the delay.
