As part of a preliminary study for a service improvement proposal, could I request the following information for the period of April 2017 to as recent as possible: 

1) By month and county, the total number of Code 17 Falls attended for patients aged 65 and older and how many SPOC Falls referrals are made

2) By month and county, the number of EPCR and paper PRF records completed

    Please note we are unable to breakdown the number of paper and electronic patient care records by county. The records are held databases and cannot be split by county; in order to establish the county, the Trust would have to manually look through each individual record to determine which county the records correlate to. The Trust has calculated that this would take longer than the 18 hours to extract and collate the information you require, therefore this would exceed the appropriate limit and as such we are unable to answer this part of your request under s.12 of the FOIA.
