I am writing to you under the Freedom of Information Act 2000 to request the following information regarding the risk summit held because of the perceived issues over the winter period.

Could you please answer the following:

*Could I have the minutes of the risk summit hosted by NHS England on Tuesday 30th January.

*Could I have the full independent report into your operational management during the winter following allegations of avoidable patient harm and death.  This is the review that Sarah Boulton announced she was jointly commissioning with NHS Improvement.  This is the report that Ms Boulton was commissioning a senior member of a different ambulance trust to investigate.

*Could you tell me which senior member of another ambulance trust has been commissioned to investigate/head up the above review.

*Could you tell me if any patients are believed to have died because of delays to ambulances being able to get to patients over the winter time period featured in the review/risk summit.

    The risk summit was hosted by NHS England and as such, the minutes of the meetings are owned by NHSE. You will therefore need to redirect your request to:  https://www.england.nhs.uk/contact-us/foi/

     
    *Could I have the full independent report into your operational management during the winter following allegations of avoidable patient harm and death.  This is the review that Sarah Boulton announced she was jointly commissioning with NHS Improvement.  This is the report that Ms Boulton was commissioning a senior member of a different ambulance trust to investigate.

    The governance review commissioned has not yet concluded, with the review not due for completion until the end of May. Upon completion, the governance review will be published via our governance processes through Trust Board and as such, a copy will be available in the future, on the Trust’s website (Board papers).  In light of this the Trust is applying exemption s.22 (information intended for future publication) of the Freedom of Information Act to this part of your request.

     *Could you tell me which senior member of another ambulance trust has been commissioned to investigate/head up the above review.

    An independent company are undertaking the review, with a senior lead in another ambulance service identified by NHSI to oversee the review process. We confirm that the names of individuals involved will be cited in the report, which will be published via our governance processes in the future, on the Trust’s website (Board papers).  In light of this the Trust is applying exemption s.22 (information intended for future publication) of the Freedom of Information Act to this part of your request.


    *Could you tell me if any patients are believed to have died because of delays to ambulances being able to get to patients over the winter time period featured in the review/risk summit.

    Independent harm analysis is being undertaken in relation to the serious incidents pertaining to delays over the festive period; this process has not yet concluded. Once this analysis is complete, the results will be discussed with NHS England and then will be published via our governance processes through Trust Board and its associated committees and as such, details will be available in the future on the Trust’s website (Board papers).  In light of this the Trust is applying exemption s.22 (information intended for future publication) of the Freedom of Information Act to this part of your request.
