    If one of your paramedics has not signed the PGD, would the typical response by them, when encountering the need for TXA use, be to call out another paramedic to scene? Can you estimate how frequently this may happen?

    The monthly and annual Callout counts for the categories ‘Assault/ Sexual Assault’ and ‘Stab/ Gunshot/ Penetrating Trauma (Incident Response Count)’ as well as Callout counts as a whole for the Ambulance Trust are really useful.

    However the spreadsheet published only allows 2018 data to show. Would it be possible to get those counts for the previous years so can see the trend in callout counts? I would like the calendar years to include 2014; 2015; 2016 and 2017.

    I would also like to know if any further information is available about the 2 adverse outcomes which arose as a result of the private taxi being used please? I.e. reviews/ investigations/ outcomes

    Further information regarding the incidents: 1 - Taxi booked for a patient but, when the taxi arrived at the patient’s address, they did not get an answer at the door nor did they update the ambulance operations centre of the situation. When the duty manager contacted the taxi company, this information was conveyed as well as the fact that the driver had left the patient’s address. Ambulance sent to address to convey patient; no harm identified. Internal information passing process changed as a result.

    2 - Patient and next of kin en-route to hospital in taxi arranged by EEAST. It was reported by the patient that the driver was using their mobile phone whilst driving to the hospital. The investigation highlighted that the patient was diverted to a different hospital and the driver carried out a phone call on their Bluetooth device to update their control of the change of destination. It was also identified that the satellite navigation system was updated whilst the vehicle was stationary. No harm identified.
